RE: Voluntary Recall of Firex® branded 10000 series and 12000 series alarms

Dear Distributor / Contractor:

This letter is to advise you that on May 15, 2008, Maple Chase Company, manufacturer of Firex® branded safety products, announced a voluntary recall in conjunction with the U.S. Consumer Product Safety Commission (CPSC) of its model COE 10000 series of carbon monoxide (CO) alarms and its model FADCM 12000 series of combination smoke/CO alarms. Item numbers include: 10000, 12000, 12000C, 12000-6, 12200, 12220, 12400, and 12400C, collectively referred to as the “Affected Models.”

A copy of the joint Press Release issued by Maple Chase and the CPSC is enclosed. This corrective action is being taken because Maple Chase has determined that the Affected Models can sound a “double chirp”/fault alarm in the presence of CO, prior to going into full alarm. Upon hearing a double chirp, the Owner’s Manual instructions recommend a consumer take the alarm out of service, which could expose consumers to hazardous levels of CO and suffer injury or death.

This recall applies only to units that were manufactured by Maple Chase between June 1, 2007 and February 1, 2008. A date code appears on the back of each unit. The date code format is a four-digit year, three-digit month, and numerical day. For example, for a product manufactured on June 1, 2007, the date code reads, “2007JUN1.” The product’s manufactured date code also appears on its master carton in an eight-digit format. For example, product manufactured on June 1, 2007 reads 02007152 on the master carton.

We are asking you to forward a copy of the Press Release to all of your known customers and occupants of residences in which Affected Models may be installed. You may also contact our Recall Center at 1-888-879-3906 with your customers’ details and Maple Chase will notify them directly. All customers and end users currently in possession of Affected Models should follow the instructions in the Press Release in order to receive a free comparable replacement product.

This recall is conducted in cooperation with the U.S. Consumer Product Safety Commission who will monitor the effectiveness of this program.
